You can get Twist of Fortune by playing Bladur’s Gate 3 until you enter the Shadow-Cursed lands in Act 2, then explore until you find the Reithwin Tollhouse. The Tollhouse is located in the western half of the map, just a bit northeast of Moonrise Towers. Inside, on the second floor you’ll find what looks like a large woman made of gold clanking around. There is Gerringothe Thorm. When you approach/get too close, you’ll automatically trigger a dialogue scene with her.
